CASE MANAGER

The Case Manager will provide support to individuals and their families through advocacy, coordination, and linkage to community resources and support services. This position also provides discretionary and independent judgment in operating practices, such as individual advocacy groups, agency quality improvement, and/ or after hour emergencies. This position is responsible to the Director of Day Services.

Salary is commensurate with education, qualifications, and work experience. Benefits include paid holidays, vacation and sick leave, and a comprehensive benefits package.

COMMUNITY ACCESS GROUP INSTRUCTOR

Under general supervision, performs work of moderate difficulty connecting adults with developmental disabilities
to a wide array of community volunteer and enrichment activities. Documents individuals’ progress on goals

COMMUNITY EMPLOYMENT SPECIALIST

Coordinates services for individuals in the development and implementation of integrated community employment based on individual needs and preferences. Provides education and consultation to businesses about hiring, training, supervising and supporting adults with disabilities. Maintains statistical data regarding various areas of employment. Provides Job Coaching and long term follow up.

FULL TIME LINE COOK / INSTRUCTOR

Responsible for the hands-on work associated with food preparation in conjunction with and under the guidance of the Catering Manager. Provides training in food services for adults who have developmental disabilities with an emphasis on expansion of vocational skills. Responsible for maintaining documentation of progress for individuals served. Rate of pay is commensurate with experience.

Provides direct care to adults with severe and multiple disabilities, keeps records, assist persons served in all areas of daily living. Rate of pay is $10.50 per hour.

PART TIME VEHICLE OPERATOR

Vehicle operator is under the direct supervision of the Transportation Services Manager. The driver operates a van vehicle to transport individuals with disabilities from home to the training center (and return) and performs other transportation services in accordance with established policy and procedures.
